Case of the week 137  ( October 2013 )
Exotropia and hypotropia of the blind eye


  19 years old patient complains of outward deviation of the right eye he suffered since early childhood, no history of previous therapy, vision in this eye is 2/60 with no significant refractive error, fundus is normal, Apart from motility disorders, the Lt eye is totally free.

     
     
     

Surgery for aligning a non-seeing eye should be restricted whenever possible to the poorer seeing eye if the angle of deviation could be corrected by working on one eye.

A logical choice for surgery for this huge exotropia and hypotropia would be:
1) Bilateral lateral rectus Recession 8.0 mm with 1/2 muscle width up shift in the right eye.
2) Resection of the right medial rectus 8.0 mm with 1/2 muscle width up shift.

The upshift of both of the horizontal recti in the right eye will have good effect on elevating the hypo deviated right eye.
