Case of the week 154  ( January 2014 )
Monocular Nystagmus

The mother of this 8 months old infant noted that the right eye of her baby is dancing. She noticed that abnormality shortly after birth . No history of delivery troubles , the baby looks fine with no apparent neurological or any other disorders.

Causes of Monocular Nystagmus in infancy

  • Tumors of optic nerve, chiasma, midbrain, 3rd ventricle or brainstem

  • Nervous system disease such as meningitis, syphilis and epilepsy

  • Superior oblique myokymia

  • Spasmus nutans

  • Unilateral defective vision

Although monocular nystagmus, is usually a benign entity, it may be associated with any of the above serious conditions. These anomalies occur infrequently but I used to advise neuroimaging in each case.
