Case of the week 164  ( March 2014 )

Esotropia and hypotropia of the blind eye




This 18 years old patient had a traumatic macular scar of the left since she was a child. She never regained good vision. Now she has an edotropia and hypotropia of the left eye. She would like to have her eyes ‘straightened.’

Vision: OD 20/20, OS light perception
Refraction: OD plano 
               OS +1.00



Esotropia and hypotropia of the blind eye

Aligning a non-seeing eye is a legitimate undertaking using the dictum ‘every humans has the right to look like a human’. Surgery should be restricted whenever possible to the poorer seeing eye. A logical choice for surgery would be:
1) Resection of the left lateral rectus 8.0 mm with 1/3 muscle width upshift
2) Recession of the left medial rectus 5.0 mm with 1/3 muscle width upshift

The upshift of both of the horizontal recti in the left eye will have some effect on elevatinging the hypo deviated left eye.
