Case of the week 185  ( May 2015 )
XT and Hypertropia



58 years old lady has had phaco cataract surgery for both eyes since about 1 month. She stated that she had Lt mature lens opacity many years ago, the XT and the hypertropia of the Lt eye appeared since 6 months. Post op. she can fixate by both eyes. VA is 6/9 OD, and 6/9 OS.
She asks for surgical alignment of this ugly deviation

   
   


 

XT and Hypertropia

A logical choice for surgery would be:
1) Recession of the left lateral rectus 8.0 mm with 1/2 muscle width down shift
2) Resection of the left medial rectus 8.0 mm with 1/2 muscle width downshift
3) Recession of the Right lateral rectus 8.0 mm.

The downshift of both of the horizontal recti in the left eye will have some effect on lowering the hyper deviated left eye. The inferior oblique is not the most effective muscle to weaken for treating this kind of hyperdeviation.
