Case of the week 28 (
January 2012
) Right Duane III pre and post operative
This 6 years old girl has been noted by
her parents and her friends to have an eye that
wanders out . The condition started shortly after
birth .
Past history is insignificant , the two eyes show
emmetropic refraction and normal fundus

Motility of this girl
has the following features :
1. Primary position Right XT
2. Rt limited adduction
3. Rt minimal limited abduction
4. Rt upshoot in adduction
5. Rt narrowing in adduction
This girl has Rt. Duane II to III or ‘Exotropic Duane’ with a
prominent upshot in adduction , both make this girl
a candidate for surgical interference .
Note that the main problem with her is appearance is
in far Lt gaze seen in picture 6. Patients with
Duane can be made betterbut not ‘perfect’.
It has been my experience that children with Duane
syndrome seldom if ever complain . It is well
understood that Duane cannot be ‘cured’ only made
some better. This is achieved by normalizing
Abnormal head posture if present , reducing or
eliminating the primary position deviation, and
increasing the field of single binocular vision.
Surgery
on the Rt eye was done consisting of a recession of
the Rt lateral rectus with a ‘Y’ split . This
perfectly corrected the XT in the primary position
and the upshoot in adduction
