Case of the week 85 ( November 2012 )
Undercorrected Exotropia

20 years old patient was
operated for outward deviation of the left eye 3
months ago, the left eye showed the same outward
deviation immediately post operative.
Examination reveals : Left XT about 70 PD, cojunctival scars denote probable surgery of the
left MR and LR. |

In an exotropia with a large angle of deviation,
straightening the eyes with strabismus surgery
is more difficult and undercorrection remains
common.
However, secondary surgery on the deviated
eye for undercorrected XT presents a challenge
because an adequate surgical dosage could not be
applied for this situation which makes the
results less predictable than those for primary
surgery. Besides, medical reports of the first
surgery in these patients are often unavailable
which makes it even more difficult to decide the
surgical amount before surgery, lastly it is
sometimes impossible to re-recess or re-resect
adequately according on the already
recessed or resected muscles.
For me, I have many successful outcome on
re-resection of the medial rectus muscle of the
deviated eye in cases of undercorrected XT like
this patient who undergoes surgery on the MR
for the previously operated eye.
